All forms must be completed and brought on the first day of camp. The following forms are required for all Katydid camps and are included in the Fairfax County Camp Forms and Parent Handbook link below:
- Pick-Up Authorization Form,
- Emergency/Medical Information & Parent Agreement Form, and
- Rules of Conduct Form.
In addition, the All About Animals and Adventure on the Farm camps at Frying Pan Park require the camper’s Immunization record from the doctor and proof of identity. Proof of identity can be an original birth certificate or passport.
- FAIRFAX COUNTY CAMP FORMS AND PARENT HANDBOOK – Download. This includes Pickup Authorization form, Emergency/Medical Information & Parent Agreement form, and Rules of Conduct form. This also includes requirements for Immunization record and proof of identity as well as COVID information.
- MEDICATION AUTHORIZATION FORM – Download. This form is required if your child requires medication administered during camp – including Epi-pens. This requires the doctor’s input and signature.
