Preschool Paperwork

Please bring all Preschool paperwork & your child’s proof of identity to our Welcome Picnic on September 7th.  If you can’t make the picnic please bring on your first day of class.  See you soon!   All required paperwork can be found here on our Preschool page – click on Registration and Forms.  You will need the Pickup Authorization Form, the Emergency Medical Release Form, and the Health Information Form.  The Health Information Form must be signed by the doctor and includes your child’s immunization record.  One additional item we need for preschool is your child’s identification record (e.g., birth certificate or passport).  We do not need a copy – we just need to see it, so please bring this as well.
